Rotator cuff tears happen easily in high-impact sports like baseball and rugby. Healing after a shoulder surgery takes a long time.
Medroot Medical explains the full recovery process and how a shoulder abduction pillow orthosis keeps your shoulder safe.
What is a Rotator Cuff Tear?
The rotator cuff is a group of 4 tendons that hold your shoulder joint in place. When these tendons tear, you need proper medical care and surgery to fix them.
6 Steps of Shoulder Surgery and Recovery
- The Surgery Phase
Surgeons repair the torn tendons in your shoulder. During this process, they use professional tools. These include arthroscopes (cameras), sterile surgical packs, suture anchors (shoulder anchors), high-strength threads, tendon patches, etc. - Shoulder Immobilization (Weeks 0 to 6)
You must wear a shoulder abduction pillow orthosis to keep your arm in a safe position.
- Normal Recovery: You need to wear the shoulder abduction pillow for at least 6 weeks.
- Severe Tears: If you have a massive tear or weak tendons, you may need to wear it for about 8 weeks.
- Key Tip: Be patient during this early stage. Do not stop using your shoulder abduction pillow orthosis until your doctor tells you it is safe.

- Starting Passive Range-of-Motion Exercises
Once your doctor checks your shoulder with an ultrasound, MRI, or physical checkup, they will allow early passive motion exercises.
- Early Movements: You will start with passive, low-load movements like pendulum exercises, as well as moving your elbow and wrist.
- Warning: Never force your shoulder to lift weight or move on its own early on. This protects the surgical stitches from tearing.
- Switching to a Shoulder Sling
When medical scans show your tendons are starting to heal well, your doctor may let you use a standard shoulder sling or strap.
- Mix and Match: Switch between the abduction pillow orthosis and the simple sling slowly. Do not throw away the pillow immediately.
- Safe Use: You can wear the simple sling while sitting or resting. You must still wear the thick abduction pillow orthosis when walking or going outside.

- Increasing Training Intensity
Your doctor will check your tendon healing progress via ultrasound or MRI. Based on these clear results, you can slowly start harder physical therapy exercises with a trainer or doctor. - Final Healing Phase (Full Recovery)
When ultrasound or MRI confirms your tendons are completely healed, a shoulder abduction pillow is no longer mandatory for your health care.
- Daily Support: You can still wear a light shoulder strap to reduce everyday shoulder pressure.
- Warning Signs: See a doctor immediately if you feel constant sharp pain, worse pain after moving, arm numbness, or if your wound leaks fluid or feels hot.
